Waiver/Policy Detail: Liability Waiver

In checking the box below I agree that Elevate Studios is in no way responsible for the safekeeping of my personal belongings while I attend class. I understand that classes at Elevate Studios may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Elevate Studios, directors, shareholders, instructors or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.

As with any form of exercise you should consult with your doctor before participating.

APPEARANCE RELEASE: I understand that Elevate Studios produces promotional material from time to time and I understand that I may be included in videos or photos taken during class practice and events. I hereby grant Elevate Studios, its sponsors and all media the right to photo or video myself and further utilise my face, name, likeness, voice and appearance as part of their program in all advertising without reservation or limitation.

Waiver/Policy Detail: Tuition Policy

All classes are paid for in advance unless you have auto pay set up with us. Any overdue costs may incur a late penalty fee if not paid by their due date. No makeup classes are given for any missed classes unless a class has been cancelled by Elevate Studios. No refunds of any kind. In the event that Elevate has to cancel a class a class credit will be added to your account if a makeup class cannot be scheduled.

I understand that when enrolling for classes at Elevate Studios I am responsible for the full payment of all classes I have signed up for, whether I finish all the classes or not.

I understand that there are no refunds or credits given for classes that I cannot attend for any reason.

Convenience fees of 3.7% + .30c will be added to all debit/ credit card payments, 2% + .40c convenience fee will be added to all gocardless/direct debit payments.

$25 late fee applies after 7 days past due. Payment plans are exempt from late fees.

Auto-pay agreement

I agree to pay an automatic payment either weekly, fortnightly or per term until the whole amount is paid in full for each term for the whole duration that myself/my child is registered with Elevate Studios Ltd in accordance to the pricing I signed up for at enrolment of each class.

I understand, have read and agree to all convenience fee charges, late fees or failed payment charges set forth in the gocardless auto pay form and the Elevate Studios enrolment fees.

Convenient fee per transaction (gocardless/direct debit) 2% + .40c

Convenient fee per transaction (debit/credit card) 3.7% + .30c

Failed payment penalty - $25 for all failed payments that cannot be re-processed within our 3 day failed payment retry.

All classes must be paid for by the 1st class of each block unless you are on an unlimited class membership or payment plan. A 2% + 0.40 convenience fee or 3.7%+0.30 depending on how you pay is charged at check out by Class manager. (Class manager- is our booking provider)

All classes are done in either 5 or 10 week blocks. 5 week blocks are for introductory classes. Levels 1-6 are by 10 week blocks or unlimited class memberships are available upon request. Pro-rated terms - all accounts will be pro-rated in line with any short terms scheduled.
