ACCIDENT AND INJURY DISCLAIMER AND CONSENT

I understand and accept that Upbeat Dance Company and its teachers are not responsible for any injury or loss/damage of property to any person during lessons and performances in the class venue, theatre or at any other event.

  • I fully understand that dance is a physical activity and can have the potential to cause injury. I agree that the company and instructor is not liable for any injury which may occur and I give permission for the instructor to administer first aid to myself (if you are the dancer) or my child if necessary.  Please be assured that students are correctly warmed up by their instructor at the beginning of each session and at the end of the session there will be a cool down. All of our instructors are first aid trained and undergo regular certified refresher training. Details can be found in our policies and procedures. 
  • I do not know of any medical reason why my son/daughter, or myself (if you are the dancer) can not take part in physical exercise and I will personally update any changes to their medical information on this portal.

PLEASE ENSURE ANY DANCERS WHO REQUIRE MEDICATIONS WHICH MAY BE REQUIRED DURING A DANCE LESSON OR PERFORMANCE, HAVE THEIR MEDICATION WITH THEM AT ALL TIMES AND NOTIFY THE INSTRUCTOR OR CHAPERONE.


PAYMENT CONTRACT FOR UPBEAT DANCE COMPANY MEMBERS 

  • Monthly fees are to be paid on 1st of every month by standing order.
  • I understand and agree that this is a membership and that Upbeat Dance Company will not accept missed or reduced payments for sickness and holidays.
  • I understand my student’s membership may be cancelled if payments are regularly late or missed.
  • I understand and agree that the membership is calculated over the year and the weeks we are closed does not affect the monthly fee. (Expect for August).
  • Classes will run through the majority of half terms/holidays unless notified otherwise. 
  • Classes will no longer continue throughout August and membership fees will be paused by the company principal. 
  • I agree to give one full month's paid notice if I decide to leave the company and cancel my membership.


UNIFORM POLICY

  • Students of Upbeat Dance Company are expected to wear their company t-shirt to every class and rehearsal.
  • Sensible and secure shoes such as trainers, plimsoles or jazz shoes must be worn to class. 
  • Dancers will be asked to remove shoes which are not considered safe to exercise in.
  • For certain classes such as acro or tumble, students will need to have their hair securely tied and secured away from the face. 
  • Jewellery is not permitted during any class unless it is a small stud piercing.


 ANTI-BULLYING POLICY

At Upbeat Dance company we run a strict anti bullying policy and will not accept any behaviour which may upset others amongst our students and staff. You can find more details in our policies and procedures.

  • I understand that bullying will not be tolerated under any circumstance and that any concerns will be handled with in line with our safeguarding policy. 
  • I understand that if we witness a student displaying behaviours regarded as bullying, they may be removed from the company with immediate effect.
  • I understand that I can raise any concerns confidentially with Amber, the Principal of the company, at a suitable time after or before a class.


SAFEGUARDING AND SAFE TOUCH

Upbeat Dance Company and it’s staff is committed to providing a safe, respectful and inclusive environment for everyone involved in our activities. 

  • I acknowledge that all staff of Upbeat Dance company have undergone safeguard training which is regularly updated and have been provided with the company policy. 
  • I understand that I can raise any concerns I may have to Upbeat Dance Company's safeguarding lead: Amber Thompson.  
  • I recognise that dance may involve physical contact between students and between instructors and students. (Our safe touch procedure is detailed in our safeguarding policy).
  • I agree that physical contact can be used on myself if my student where appropriate (see our safeguarding policy).
  • I understand that myself or my student can say no to being physically touched at any time and the instructor will ensure their wish is fulfilled.
