Terms and Conditions
DECLARATION: By electronically signing below, I confirm that I have read the document in its entirety. I understand the document, have had an opportunity to ask questions, and agree to be bound by its terms.
PHOTO RELEASE: We will be taking photos and videos during classes, events, exams etc. Anita Arora Dance Company reserve the right to take and use photos of students, teachers, and studio activities on its website and printed materials. I understand that these pictures will be accessible to anyone with internet access. I give my permission for Anita Arora Dance Company to use photos of me/my child on its website, on Facebook, social media, in printed materials, and in electronic forms of communication. MEDICAL RELEASE: I give my permission for Anita Arora Dance Company to transport, or call an ambulance, to take me/my child to a medical institute, if necessary. In case of emergency, if none of the emergency contacts respond, I hereby give my consent for emergency medical care to be prescribed by a duly licensed Doctor of Medicine.
TUITION AND FEES: By signing this document, I understand that the fee is charged as a monthly direct debit and I agree to a minimum of one month. I acknowledge that if I withdraw from classes at any point, I must notify Anita Arora Dance Company at least one month prior to the date of withdraw.
ATTENDANCE: It is the students responsibility to attend all classes within the term. Refunds are not offered if you miss any classes and are not carried forward. If you are unable to attend due to medical issues, a doctors note must be provided and the fee will be adjusted when the student is ready to resume lessons. Anita Arora Dance Company reserves the right to alter times and dates of classes or venue due to situations beyond control.
WAIVER AND RELEASE: I agree that my/my child's engagement in physical activity is my decisions on the premises or any venue where we participate as a representative of the Anita Arora Dance Company we do so at our own risk. I agree that we are voluntarily participating in activities and use of said facilities, premises (including the parking lot) and designated venues. We assume all risk of injury, illness, damage, or loss to us or our personal property that might result, including, without limitation, any loss or theft of any personal property. I agree that this consent and assumption of risk statement covers each and every event or activity sponsored by Anita Arora Dance Company. I agree to release and discharge you (and your affiliates, employees, volunteers, assistants, agents, representatives, successors, and assigns) from any and all claims to cause of action (known or unknown) arising out of your negligence. I acknowledge that I have carefully read this Waiver and Release and fully understand that it is a release of liability. I am waiving any right that I may have to bring legal action to assert a claim against you for negligence.
DISMISSAL FROM CLASS: Anita Arora Dance Company provides a fun, rewarding, and challenging experience in dance to students of all ages and abilities. This results from the collaboration among parents, students and teachers. Class participation, a positive approach to learning, adherence to policies, and parental support are important factors influencing the experience of your student and others enrolled in class. In order to maintain a welcoming atmosphere, Anita Arora Dance Company reserves the right to dismiss any student without a refund if any misbehaviour is seen.
