Terms and Conditions

We realise that participation in dance classes and activities could involve possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Centre Stage Dance Academy. 

I/we agree to release Centre Stage Dance Academy including its teachers, dancers, volunteers and facilities used by entities from any cause of action, claims or demands now and in the future. I/we will not hold Centre Stage Dance Academy liable for any personal injury, or personal property damage, which may occur on the premises before, during and after classes. Furthermore I agree to obey the class and facility rules (Centre Stage Dance Academy Code of Conduct) and take full responsibility for my/our behaviour in addition to any damage we may cause to the facilities utilised by Centre Stage Dance Academy.
