I/ we realise that participation in dance classes and activities could result in some possible personal injury. Despite precautions taken by the studio, accidents or injuries may occur.

BY SELECTING "I AGREE" OR SUBMITTING THIS FORM, I/WE (parent/ guardian) AGREE TO ALL TERMS, DISCLAIMERS, POLICIES, AND ASSUME ALL RISK RELATED TO THE USE OF ANY AND ALL SPACES USED BY JL DANCE PRODUCTIONS.

I/we agree to release from responsibility JL Dance Productions PTY LTD including all teachers, dancers, students, staff members, contractors, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold JL Dance Productions PTY LTD including all teachers, dancers, students, staff members, contractors, and facilities liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions, or death, or any personal property damage/ loss, which may occur on the premises before, during, or after classes.

Furthermore, I/we/student(s) agree to obey the class and facility rules and take full responsibility for my/our/student(s) behaviour in addition to any damage I/we/student(s) may cause to the facilities utilised by JL Dance Productions.

PHOTOGRAPHY/ VIDEO
I give permission for my student(s) photographs/ videos and their likeness to be used for publicity and advertising in the studio, on the JL Dance Productions website, on social media, in the community.

If you would like to revoke your photography and video consent please enquire with a JL Dance Productions team member. We will provide the appropriate release form. 

SAFE PLACE POLICY - ZERO TOLERANCE POLICY
JL Dance Productions PTY LTD is a safe and supportive environment. We have a Zero Tolerance Policy for bullying, fighting, hitting, verbal or physical abuse, or any content or action intended to cause harm either physically or emotionally. This is applicable to students, parents/ guardians, or anyone within or in the immediate vicinity of our premises. This is also applicable to our online digital communities. 

In the event that you, as a parent/ guardian, or your student(s) feel uncomfortable or have a concern, please bring the issue to the attention of Dane, Jess, or the Office Manager immediately. DO NOT take it upon yourself to resolve the issue. Doing so may inadvertently place you in violation of our Zero Tolerance Policy.

‘Feet on the Floor’ Policy: JL Dance Productions PTY LTD has a strict ‘Feet on the Floor’ policy. This policy is in place for the safety of the students and to reduce the possibility of accidental injury. This means no student is to do any trick or acrobatic element of any kind without approval from the teacher. (eg. No handstands around others or while waiting in line.). This policy also extends to keeping your hands to yourself, rough-housing and running in class. Students are made aware of this policy in class. Failure to adhere to this policy, or in the event of accidental injury to one’s self or others due to not adhering to the policy will result in the student being asked to sit and take a 5-minute reset break either in the studio or in the lobby. This 5-min reset break is not a punishment for your student, but a reflection on the seriousness of safety for everyone. Consistent or repeated instances will result in the student being in violation of our Safe Place Policy.

TERM FEES

Term Fees are Not Refundable.

There is a $25 annual Registration Fee per student.

Term Fees are not offered on an a la carte/ pay-as-you-go basis. When you enrol in any class you are agreeing to pay the full term fees.

• In rare cases, and to the sole discretion of the Director, exceptions can be made. Possible examples of exception to this rule may include, but are not defined as, injury or interstate/ international relocation. NOTE: Choosing to not return for any reason is not considered an exception to the rule.

Payment plans must be paired with an active GoCardless mandate which allows JL Dance Productions to auto-debit instalments on their due date.

Term fees must be paid in full or have an official payment plan setup by a JL Dance Productions team member before your student(s) first class.

• The one exception to this is if you enrol in a class or are joining JL Dance Productions after a term has commenced. In which case, your invoice must be paid in full by the invoice due date or have an official payment plan setup by a JL Dance Productions team member.

Failure to settle your invoice in full by the due date or failure to keep your payment plan current will result in your student(s) not being allowed to join class until your invoice is settled in full or payment plan is made current. 

CLASS PACKAGES AND DISCOUNTS

Packages cannot be combined with any other discounts.

All discounts are applied to class prices before GST is applied.

LATE FEES/ ADMIN FEES

A 10% admin fee will be added to any outstanding invoice. 

Invoices have a 7-day grace period from their due date before late fees are applied. 

Please settle your invoice by the due date or contact us to set up a payment plan to avoid these fees.

Payment Plans that do not remain current will incur late fees. In the event multiple late fees are applied to an outstanding invoice the late fees are calculated and compounded on the outstanding balance per late instalment.

Any invoice not paid in full by the end of the current term will incur an additional 20% admin fee on the remaining balance.

• If an invoice remains unpaid at the conclusion of the subsequent term the invoice was originally issued during, the invoice will incur an additional admin fee equal to 400% of the outstanding balance.

PERFORMANCE SQUADS

These classes are invite only. Please enquire if you are interested. Performance Squads will partake in various performance experiences throughout the year, ranging from dance competitions to community events. There will be additional costs throughout the year for competition entrance fees, costumes, etc.

8&U Performance Squad dancers must take at least x1 ballet or contemporary/lyrical class per week in addition to Squad Class.

Free trials are not offered for Adult classes or Private Lessons.

Term Fees are not refundable.

All Fees and Sales are final and not refundable.

Registration Fee is waived for Adult Ballet and Dance & Play.

I understand by selecting "Agree" and creating a client account or physically signing this agreement in the event of an in-person registration, I agree to all terms above.
