Health Declaration: I hereby declare that I am in good physical condition and have no medical conditions that would prevent me from participating in martial arts training.

Safety Guidelines: I agree to follow all safety guidelines and instructions provided by the instructors of Martial Art Hastings Ltd.

Criminal Record Declaration: I confirm that I have not been convicted of any criminal offence, and I currently have no criminal charges open against me.

Fees and Testing: I understand that fees for training and testing may apply and will be communicated by Martial Art Hastings Ltd.

Photography Release: I acknowledge that there may be occasions when photos are taken during training sessions. For individuals under the age of 18, parental or guardian consent is required for any photos taken. These photos will only be used for promotional purposes by Martial Art Hastings Ltd.

Code of Conduct: I agree to adhere to the code of conduct set forth by Martial Art Hastings Ltd and will treat all members, instructors, and staff with respect.

Right to Refuse Entry: Martial Art Hastings Ltd reserves the right to refuse entry and participation in training or practice if it deems an individual unfit to do so.

Membership Renewal: I understand that membership fees must be renewed annually
