MC Dynamic Studios Waiver and Liability Release Form

This Waiver and Liability Release Form (“Agreement”) is entered into between the undersigned participant

(“Participant”) or, where applicable, the Participant’s parent or legal guardian (“Guardian”), and MC Dynamic

Studios (“the Studio”). This Agreement is effective as of the date signed below.

ASSUMPTION OF RISK

I understand that participation in dance classes, workshops, rehearsals, performances, private lessons, and

related activities, whether in person or online (“Activities”), involves physical activity and a risk of injury. I

voluntarily choose to participate and accept responsibility for my participation.

WAIVER AND RELEASE

To the extent permitted by law, the Participant releases MC Dynamic Studios and its owners, directors,

employees, contractors, and agents from liability for injury, loss, or damage that may arise in connection with

participation in the Activities.

AUSTRALIAN CONSUMER LAW

Nothing in this agreement is intended to exclude, restrict, or modify any rights or remedies that cannot be

excluded under the Australian Consumer Law or other applicable legislation.

MEDICAL RESPONSIBILITY

The Participant confirms they are physically and mentally able to participate and have disclosed any relevant

medical conditions. The Participant understands that MC Dynamic Studios does not provide medical care and

agrees to be responsible for any medical costs incurred.

RULES AND CONDUCT

The Participant agrees to follow all Studio rules, policies, and safety instructions. Failure to do so may result

in suspension or termination of participation.

PERSONAL PROPERTY

MC Dynamic Studios is not responsible for loss, theft, or damage to personal property brought onto the

premises or used during participation.

CLASS SCHEDULES AND CHANGES

The Participant acknowledges that class schedules may be subject to change, and that MC

Dynamic Studios reserves the right to modify or cancel classes due to unforeseen circumstances,

such as instructor availability or weather conditions.

DISABILITY ACCOMMODATIONS

If the Participant has a disability and requires accommodations to participate in the Activities, the

Participant agrees to notify MC Dynamic Studios in advance so that reasonable accommodations

may be arranged. MC Dynamic Studios is committed to creating an inclusive, supportive

environment where all dancers of all abilities can thrive.

ACKNOWLEDGMENT OF RISKS SPECIFIC TO DANCE

The Participant understands that dance involves activities such as jumping, stretching, balancing,

and pivoting, all of which can lead to falls, overuse injuries, or strains. The Participant accepts these

risks as inherent in the nature of the sport and agrees to participate voluntarily.

HEALTH INSURANCE REQUIREMENT

The Participant acknowledges that it is their responsibility to have adequate health insurance

coverage for any injuries that may occur during participation in the Activities. MC Dynamic

Studios is not responsible for any medical expenses incurred due to injuries sustained during the

Activities.

ONLINE / VIRTUAL CLASSES

For online or virtual classes, the Participant is responsible for ensuring a safe space and appropriate setup.

PHOTO AND VIDEO RELEASE

The Participant grants permission for MC Dynamic Studios to use photos or videos taken during participation

for promotional or instructional purposes. You may withdraw consent by notifying the Studio in writing.

Personal information is collected and handled in accordance with the Privacy Act 1988 (Cth) and the Australian

GOVERNING LAW

This Agreement is governed by the laws of Queensland, Australia.

PARENT / GUARDIAN CONSENT (FOR MINORS)

If the Participant is under 18 years of age, the undersigned parent or legal guardian confirms authority to

enter into this Agreement on the Participant’s behalf.

ACKNOWLEDGMENT

By enrolling, I acknowledge that I have read and understood this Agreement and agree to its terms.
