PAYMENT MUST BE MADE AT THE TIME OF YOUR BOOKING
PAYMENT – Full payment is the Only way to secure your booking. Full Payment must be made at the time of booking to secure your child’s attendance. Non-Payment will result in your child being removed from our rolls and they will not be able to attend.
CREDITS & REFUNDS
· We DO NOT offer refunds for change of mind or transfers on our programs once payment has been processed unless a medical certificate is supplied.
· If a participant misses any sessions due to medical reasons, they are eligible for a credit or refund upon receipt of a medical certificate by the Accounts Department.
· Fees will not be refunded for change of mind.
· Fees will not be refunded for non-attendance.
· Applicable GST.
PHOTO PERMISSION
Photo Release for Minors (if under 18yrs) Northern Districts Gymnastic Club has my permission to use my or my child's photograph publicly to promote the gym. I understand that the images may be used in print publications, presentations, websites and social media.

Parents Must sign their children In and Out on the daily roll located at Reception.
If your child is being picked up by someone other than yourself, please let our reception staff know.
We ask parents not to arrive before 7:45am
Parents are to remain with their children until coaches invite children to enter the gym 5minutes before the start time.
What to Bring - If your child is attending a full day you will need to provide morning and afternoon tea, lunch and a water bottle. Please remind your children not to eat all their food in one sitting.
If your child is only attending a half day you will need to provide morning or afternoon tea and a water bottle.
What to wear - We ask that you dress your children appropriately for gymnastics. Comfortable clothing with no zips or buckles that can get caught on the equipment and no skirts or dresses.
Girls - Leotards, shorts, leggings, fitted T-shirt.
Boys - Shorts and a T-shirt
Jewellery is Not permitted. Long hair should be tied back. No electronic devices are permitted

DISCLAIMER - We, the Pivotal Gymnastics Management, members of the body corporate (hereinafter referred to as “PGM”), advise that the learning of and participating in the sport of gymnastics, by its very nature, contains an element of risk. Whilst all due care and caution is undertaken to ensure the safety and wellbeing of gymnasts under the care of PGM, no liability is accepted for injury sustained whilst under that care. The personal information provided by you on this form will be used in accordance with our Privacy Policy. To obtain a copy of our Privacy Policy, enquire about any privacy issue, or make a request for access to information, please contact the Business Manager at PGM.
Medical Information
Medical Information - Yes - Please provide details of any medical, sport injuries, physical or intellectual conditions that may have bearing on your child's ability, safety or behaviour in class. Application For Registration
APPLICATION FOR REGISTRATION - I/ We the undersigned applicant, having read the conditions outlined, in addition to the DISCLAIMER, and agreeing to abide by the Constitution, Board Resolutions and Club Rules in force at any time, hereby apply for registration to the Pivotal Gymnastics Management. Furthermore, I authorise my child’s coach to obtain any medical assistance that is deemed necessary for myself/ my child, in the case of an emergency, and agree to pay all medical expenses incurred through the emergency. The PGM may at its discretion commence proceedings or engage debt collectors to recover fees outstanding for a period in excess of thirty (30) days. The expense of debt recovery will be a cost to the member or former member and will be added to the amount of any outstanding fees.

Recreational Programs

· Term fees will not be refunded for change of mind after enrolment.
· A member who withdraws from the program will not receive a refund of their annual membership fees.
· Those who have their membership terminated by NDGC will not be entitled to a refund.
· Fees corresponding to training already provided during a period.
· Applicable GST.
· Termination of Membership from the Recreational Program.
· A member participating in a recreational gymnastics program may withdraw from the program at any time provided written notice is given to the Accounts Manager, refunds do not apply.
Fees In Arrears
A member who has an outstanding account for a period of more than twenty-one (21) days or an invoice being due, will be issued with a suspension notice, the gymnast will not be entitled or permitted to attend or participate in NDGC gymnastics activities, including, training or competition.
A member will be entitled and permitted to resume gymnastic activities when the outstanding monies are paid, and the account is up to date.
PGM may, at its sole discretion, refuse to renew the registration of a member with outstanding debt.
Debt Recovery
