§ At all times a qualified first aider will be on site. I give permission for the seeking of any medical advice or treatment if needed.
§ I fully understand all students attending our classes will be fully supervised at all times. CCTV is in operation in all areas of our venue which is operated by Bellfield Junior School.
§ It is my responsibility to look and understand PADA policies and procedures. I understand to follow all the procedures that PADA has in place. This does include referring to “Parent & Carer Code of Conduct Policy” & “Student Code of Conduct Policy”. If any issues do arise and using these policies to help resolve a situation peacefully.
§ I understand that students aged under 13 years old must be escorted through the car park by a named responsible adult to the entrance door to hand over to PADA staff and collect them from the entrance door at the end of class. Students (under 13) must not be left unattended before handing students to PADA Staff as they are my responsibility.
§ I accept and respect the ethos, aim & curriculum of Phoenix AcroDance Academy.
§ I understand PADA is a structured academy to support the next generation of acrobatic dancers, also with basic life skills which includes, but not limited to; leadership, teamwork, responsibility, self-motivation, supporting others and assisting.
§ PADA is a privately operated business. I acknowledge my responsibility to remit all academy invoices promptly, ensuring payment by the specified due date. I acknowledge that a mandatory £5 late fee will be applied if an invoice remains outstanding for three or more days.
§ I accept that PADA reserve the right to revoke memberships and remove students from PADA at any time.
Waiver
§ PADA staff are all trained to provide all activities to minimise the risk of injuries and teach students about the risks and looking after their bodies to minimise injuries, but I understand that accidents & injuries may occasionally occur due to the nature of the activities held at PADA.
Phoenix AcroDance Academy cannot be held responsible of any accidents or injuries, this includes but not limited to; broken bones, ligament damage, brain damage, bruises, soft tissue damage, spinal damage, trips, falls, loss of balance, mental health issues (including self-harm), or death. I accept the possible hazards and possible dangers of the activities at PADA.
§ I understand it is my responsibility to inform PADA by email of any student illnesses, injuries, current / new medical conditions, self-harm (suspicion or confirmed) or suicidal (thoughts or attempts). I understand that PADA can support these students, but PADA needs communication and support from parents for PADA to being able to support these students.
§ I give permission for PADA staff, support staff, guest teachers, student teachers, assistant teachers, examiners, course conductors, students or volunteers to physically spot students in the name of safety to support technique and minimise the risk of injury while learning skills during activities at PADA.
Compulsory Conditions
§ I accept that students at PADA are required to commit to monthly homework as part of their training at PADA completing a minimum of 4 days per week commitment. This is overall it’s the student’s responsibility of their training for completing the homework. I will support the student with this and support them to return them on-time in their homework folder.
§ I understand that commitment to classes is essential for students training at PADA and compulsory for our performing teams. PADA Performing Team Agreement & Conditions Form must be signed if part of the PADA Performing Team.
§ Parents must email PADA regarding student’s class absence on the day of absence any time before the class start time for any class at PADA, including the *PADA Performing Team Sessions*. *Subject to prior approval.
I accept to email PADA of my child’s class absence on the day of absence, ideally in the morning if possible.
