Assumption of Risk
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of cheerleading, tumbling, stunting and dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/or causes of action of any kind, including any and all claims of negligence, arising as a result of such activity from with liability could accrue to Phoenix Cheer & Dance, its officers, agents, employees, instructors, owners and all affiliated entities (hereinafter collectively referred to as Academy SA Pty Ltd T/A's Phoenix Cheer & Dance).
Release of Liability
I hereby agree to release Phoenix Cheer & Dance of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in cheerleading, tumbling, stunting and dance on behalf of myself / my child. I am aware that this is a release of liability and acknowledgement of my voluntary and knowing assumption of risk of injury. I have acknowledge this document voluntarily and of my own free will in exchange for being a registered member of Phoenix Cheer & Dance. If I am a minor, my parent and/or legal guardian have acknowledged this document releasing Phoenix Cheer & Dance from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risk of injury inherent to this activity.
Medical Emergencies
I hereby authorise Phoenix Cheer & Dance owners to act on my behalf to provide emergency medical treatment. I further release Phoenix Cheer & Dance of all liabilities associated with myself or my child's attendance at Phoenix Cheer & Dance. Phoenix Cheer & Dance along with its officers, agents, employees, instructors, owners and all affiliated entities has my permission to obtain emergency medical treatment for myself or my child when I cannot be reached or if a delay in reaching next of kin would be dangerous for myself or my child. I understand that I assume all financial responsibility for any treatment or injuries sustained by myself or my child while in Phoenix Cheer & Dance care.
**Please provide up to date and correct medical information for each athlete being enrolled that will assist in the process of medical treatment if such becomes necessary including health conditions, allergies, previous injuries or physiological conditions.
Payment Policy
Phoenix Cheer & Dance Classes: Competitive Cheer teams have the option of payment per term or monthly direct debit, costs vary per team and level and can be found in the 2021 Information Pack available on request.
Non-competition Classes:
Classes are listed in 2021 information pack, payment options include casual classes, term or monthly direct debit. A non-competitive athlete registration fee will need to be paid for athletes attending these classes.
Payment Methods:
Customers can either pay per term or per month for classes via their online parent portal, all customers will have a log in on completion of this registration form, if you are not sure how to access your portal call our reception on 0400943355 or email [email protected]. All yearly accounts will be set up on direct debit option through Pay Advantage, if this option is selected in sign up then a separate email with sent with details.
Late Payments: If payment is not made on time as per our invoices a late fee will be applied at 14 days late and the athlete will not be allowed to continue training until the account is brought up to date. Arrangement for outstanding invoices is always encouraged.
It is also a requirement that all monies paid for classes, registration fees, uniforms, practice attire, choreography, competitions fees etc will not be refunded once paid to Phoenix Cheer & Dance.
Phoenix Code of Conduct
Always show respect for coaches, fellow team mates, athletes, parents, judges, officials and spectators.
Athletes are to come to training with a positive and can do attitude.
Everyone is to use appropriate language and behavior at the gym, competition and when representing Phoenix Cheer & Dance in the wider community.
No bullying allowed including but not limited to verbal, physical and cyber bullying.
Competition teams are a one year commitment and by agreeing to this code of conduct I am acknowledging to this commitment.
I will not gossip about another team member, parent, coach or gym. If I have something that needs to be discussed I will raise this with either a coach or the owners of Phoenix Cheer & Dance.
It is the athletes responsibility to know what is going on with their team. Phoenix Cheer & Dance will provide different avenues of communication to athletes and parents.
Appropriate training gear is to be worn to all training, this includes the Phoenix Training Set, Phoenix Merchandise or other training attire in club colours.
Routine and team decisions are the responsibility of the coaches and no one else.
Routines are owned by the club and are not to be shared on social media without permission.
Parents / relatives / friends and cheerleaders are not allowed to speak with competition officials for any reason.
The Phoenix logo is copyrighted. No one can reprint our logo on any apparel or other items for apparel distribution or sale. All items are to be sold through Phoenix Cheer & Dance.
Always demonstrate good sportsmanship before, during and after competition.
Treat athletes and coaches from other clubs with respect and kindness,
Vacation and excused absences must be submitted via email to [email protected].
If running late or sick you must notify your coach as soon as possible.
No jewelry is to be worn to practice or competition.
Myself / my child understand and agree to abiding by the rules of this code of conduct. If we breech any of these rules we maybe suspended or dismissed from Phoenix Cheer & Dance.
