By Ticking this box you (and/or your child/dependant) understand and are willing to abide by all Club Rules and Regulations set by Powerhouse Elite and are fully aware that Powerhouse Elite reserves the right to refuse any application at our discretion. 

You agree and give consent (for your child/dependant if under 18) to be videoed or photographed for media, memorabilia or advertising purposes. 

You also agree not to send your child if they display any cold or flu like symptoms due to the nature of COVID19.

You also acknowledge that you are fully aware of the physical nature of cheer & dance and will not hold Powerhouse Elite, its teachers/coaches, volunteers or other employees liable, in case of injury.
