Terms and Conditions

PRO Status Billing Authorization 


I represent and warrant that if I am purchasing something or paying for a service from this facility-PRO Status Houston or from other merchants through this facility that (i) any credit card or bank account draft (ACH Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.

I hereby authorize (if online payment is made or autopay information is provided) this facility to charge my ACH draft, or credit card account. I understand that a 30 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from class(es).


Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.


PRO Status Rules, Terms and Conditions


REGISTRATION FEE
All Students will be charged a $40 registration fee. Enrollees with one or more siblings will pay $60 total. This fee is good for one year.


MAKEUP POLICY
Because of our strict student to teacher ratio, missed classes will not result in make-up classes, prorated tuition or refunds. No refunds for classes missed including scheduled holidays.


DROP PROCEDURE:
PARENTS MUST NOTIFY PRO STATUS HOUSTON TO DROP A STUDENT FROM CLASS. Only a written notice via email or hand delivered to our front desk will be acceptable.

Please note: You are responsible for payment for your student's classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time you notify the staff VIA WRITTEN NOTICE. Please do not rely on your student to verbally let us know that he/she will no longer be attending classes. If a student stops coming to class without notification then that student's account will be charged for the additional 30 days. This charge will be for holding the student's place in that class instead of offering that place to one of the many on a waiting list.


WHAT TO WEAR
Boys may wear white tucked in T-shirts and black tights/shorts and Black Leotards and flesh tone convertible tights for Girls. NO chewing gum or dangling jewelry. Hair should be pulled neatly and securely away from the face so that is stays up for the entire class. Girls should not wear bows/barrettes or other large hair ornaments that may cause discomfort during activity. All students should have activity-appropriate footwear during class. (Ballet, Lyrical Shoes, Pointe Shoes, etc.) Personal items should be left at home. Jewelry should not be worn during classes. 
PLEASE LEAVE JEWELRY ARTICLES AT HOME. This facility's staff will not be responsible for ANY items that may be lost or stolen. Be sure your student's personal items are marked with their name.


ARRIVAL AND PICKUP
Be sure your student arrives 5 minutes before (no earlier please) his/her scheduled class time. Please pick up your student on time. Instruct your student to wait inside the building and you should escort them from the building to your car. During peak times the parking lot is crowded. Please take into consideration that our students may include young children. Please drive slowly and carefully. Do not take a chance on your student running to and from your car.


Waiver and Release of Liability

As an athlete at PRO Status, I realize that potential dangers exist. While in good health and able to participate in tumbling/dance activities, my participation may result in injury due to accidents or other causes unforeseen. Such injuries may include sprains, strains, fractures, and/or dislocations. By participating, I hereby and knowingly assume all risk resulting from these activities. 

On behalf of myself and the enrolled minor, I hereby agree to release, hold harmless and indemnify PRO Status Cheer and Dance Company, it’s agents, officers, and staff from any and all claims and lawsuits which may result from my participation in the above mentioned activities.


PRO Status Cheer and Dance Company is protected from any risks in delivering a professional service. Our professional liability policy protects PRO Status Cheer and Dance Company if we are sued for negligently performing a service, even if we haven’t made a mistake. I understand that PRO Status has the right to remove any parent and/or athlete from the gym at any time prior to the contract end date if the parent and/or athlete is a liability to the brand. PRO Status has a $1,000,000 coverage policy for:

• Alleged or actual negligence

•Defense Costs

•Personal/Bodily Injury

• Libel or Slander

•Worldwide Coverage

•Temporary Staff and Independent Contractors •Claims arising from services provided in the past •Claims and Damages


Athlete Protection

I also understand that PRO Status Cheer and Dance Company is owned by a licensed educator, Stacy Lockett. It is her duty to serve and protect all youth as mandated by the Texas Law Section 261.101 of the Texas Family Code. This code mandates that any individual who is licensed or certified by the state who suspects child abuse or neglect must report it immediately to Child Protective Services. If not reported, Stacy Lockett could lose her Texas Educator License.


Photography/Videotape

PRO Status and/ or any of its affiliates will possibly videotape or take photographs of your child during classes/performances for marketing purposes and social media.
