By enrolling with Strive Performing Arts, you agree:

By bringing my child/self to Strive Performing Arts, I understand that while proper care will be taken to reduce risks through preparation, training and appropriate safety measures (e.g. acro mats), there are risks of physical injury associated with dancing, acrobatics and drama.  As I recognise this risk, I knowingly and voluntarily waive all rights and causes of any kind, including but not limited to, injury and negligence while attending a Strive class or event.  I hereby agree to release Strive Performing Arts and hold Strive Performing Arts harmless of all liability and I acknowledge that I knowingly and voluntarily assume full responsibility of all physical injury arising out of active participation in dancing and acrobatics.  I have agreed to this release of liability freely and of my own free will.

I understand that at times (often in acro classes), physical touch is required between teacher and student to reduce the risk of injury and ensure the safety of the student.  I consent to Strive teachers physically spotting my dancer for skills where needed.

I understand that all fees are payable in full prior to the commencement of each term (unless on a prearranged payment plan) or a 10% late fee will be charged.  All fees are based on a 9 week term.

I understand that no refunds will be given for missed classes under any circumstance (unless absent for 3 or more weeks with a medical certificate provided).  Any classes cancelled by Strive will be credited towards the following term.
